Very strange indeed. Do you have e.g. anti virus software running at all on these systems?

It still does looks like some anomaly in Sierra. I really would like to find out more so if you could send me the outcome of plutil -p ~/Library/Preferences/com.apple.dock.plist again, but now after you have restarted the system (and with the second icon still in the dock after that reboot). That should tell us where the OS thinks PM is/was.

I attached you a new PM_Log. Problem is still the same. But it looks like a Apple bug.

Hi Dominik,

Indeed nothing out of the ordinary in your log. But have a look at this thread it is very likely also the cause of the behaviour you are seeing :)

In short: properly reinstall PM from a fresh image directly to /Applications and do not start it from the dmg as it will then get quarantined (or whatever they call it) each time you run it.

Hi Everyone,

Finally I found a way, that seems to work so far. All the time since I use PM, I created a Photomechanic folder in the Apple Application folder where I put in the Extras, ReadMe.PDF and PhotoMechanic 5 App File. Now I, moved the PhotoMechanic App File out of this folder directly into the Application Folder.

I have no issues anymore with double Icon or inactive ghost icons. This does work on a iMac 5K OS 10.12.4 and on the new MacBookPro with PM build 18359.
Hope this does help you too, if you have the same issue.
