Making a custom code replacement file

I copied and pasted the rosters of 2 HS basketball teams. Using Jason Watson's program at codereplacements.com I copied and pasted the nos. names and team names into their custom replacement window. I have included screen shots. It's not pulling  the names into PM caption field. I've done this before without problems. What am I doing wrong?

I see no screenshots.  Your code replacement text file looks fine though a bit repetitive.  What codes are you entering?  What character are you using as your code replacement delimiter?
