Duplicate files upon Ingesting

...hi
We're trying to ingest and ever after rebooting computer, restarting PM, after trying the old version of PM as well... we still get duplicates with the A attached in the filename.
(ex:  3135_0251A.nef )
How do we correct this issue?

Hi Marc!

The "Incremental Ingest: Copy new photos only"-checkbox in the ingest dialog is activated?

Sadly this was not the problem.
MacBook Pro 10.7.5
Just started this morning.
PM 5.0, build 13444 (c5a5ad3) AND PM 4.6.9
PM crashed (no idea why) and then started ingesting duplicates.
We deleted all ingested files, rebooted machine and it still happened and now every card we ingest all files are duplicated.
We tried clearing caches.
FWIW we are ingesting raw+jpeg which we are viewing as a combined preview....

probably a secondary location is added in the ingest dialog directing to the same path?

Would you mind showing a screen shot of the ingest dialog?

It definitely sounds like your Primary and Secondary destinations are the same path.  Check your Ingest settings.

.. We figure it out..
it was an oversight on our part.
we checked both Primary and Secondary destination to go on the same Drive... so therefore the duplicate files
**probably a warning prompt would be useful in the future**
