Error when Renaming

This has worked in the past, but I am now getting an error when i try to rename a file with the following string, {seqn}_{filename base:-6}. After the error occurs, if I quit PM and try to rename it again, everything works fine and I get the desired result. Usually this is a folder of 20 to 1000 images. Any help is appreciated, It is not fun having to do this over and over, reseting the number sequence each time. I am running  5.0, build 15800 (e6ecc1c) with Mac OS 10.7.5.

Here is the error:
An error occurred while renaming photo "001_68420002.jpg". The operation will be cancelled, and the original filenames will be restored.

Original filename:
001_68420002

Once the error occurs this is the name that remains:
_###_TMP_00000_001_68420002

if i run the same renaming after restarting PM, everything then works well and get this, the original desire result:
849_420002

I don't know why it would do that unless you're having renaming collisions during the rename.

Thanks Kirk, by this do you mean that some of the names would end up the same if renamed or even the sequence was not factored in? All of the files have unique names, I am just using the :-6 to preserve the last 6 digest and then want to set a new sequence. Is there another way that I do this?

I am happy to make a shots screen grab video if it would help at all.
