City, State and Country fields empty after saving from Photoshop

I shoot for a wire service and have Meta Templates for each State/Location where I shoot. I have the City, State and USA in the Ingest template. When I save an image from Photoshop, these three boxes are empty. All other info carries over. I've been using PM for 14 years and just now have this problem.

I would check the Metadata (IPTC) Info dialog in Photo Mechanic after Ingest to make sure the Metadata Template wrote the City, State and Country info.  If the info is present, then I would check Photoshop's File Info to make sure the info is still present after sending the file from PM. If it is, then how are you saving this file from Photoshop?  Does this problem happen with all files saved from Photoshop or just some?
