structured Keywording

Hi,
Can I use the 'structured keywording' pane and build in my own hierarchy of keywords. I've tried to use this but have not managed to add a new hierarchy. Is there a secret I am missing?

If you right-click on an item you'll get a contextual menu that allows you to do things like create new items, rename, delete, etc.
