Missing images on ingest!

I don't know if this has anything to do with the beta 4.6.7 beta 5 - BUT I just ingested 8 CF cards and finished with no errors. But I was missing images (lucky I noticed). On one card images from 7220 to 7301 and on a second card number 6949-7028 were NOT ingested. I was running a simaltanious ingest of 4 - 4-8 gb cards using Firewire Lexar Pro card readers.
2 of the 4 cards missed ingesting some images (I'm checking the other 2 from my last 4 I'm not sure I can still check the prior 4.)
I re-ingested but they were NOT again.
I then unchecked Incremental ingest and they seem to be ingesting.
I'm running on a Vista PC.

On a side note  - I still think PM would be greatly improved if - for any missing frame numbers PM (gave a warning or inserted a BLANK frame - It may just replace a frame I deleted in camera but if there are 100 of the in a row I would know there is a bigger problem (like this bad ingest or a lost card)
This issue realy scares me as I increase the number of cards I deal with. If this problem just missed 20-30 shots in the middle of a game I might NEVER have known about it.

I could see adding a warning with a text log that listed potentially missing sequences, but putting 'blanks' in place of potentially missing sequences would be both difficult (PM only browses what it sees in a folder) and confusing for users.

Do you always format your cards each time you insert them into your camera?

For PM to 'skip' files without reporting an error would indicate that the OS did not return the correct directory information when PM requested it.

Kirk - After color coding the 1500 + images in the directory and trying to figure out exactly which were missing. I found the images HAD been ingested. They weren't there when I color coded and they weren't there when I looked to see which frame were missing. But after reingesting the 6 availabe cards into a ne directory - as I scrolled through to see if I was missing any others I saw the 2 Blocks of images that were missing & had not been color coded.
I thought maybe I passed by them while coding before they were finished ingesting BUT they were not there when I reingested either. that's how I know which numbers were missing. I was sorted by filename. I can not think of how they were not there as I went thru over & over to check for the missing numbers.

I always format as I put a card BACK in the camera.

I would appreciate anything that could warn of MIA's

Was there anything different about those specific images?  Had you locked them in the camera?
