Cannot ingest anymore.

I have been using Photomechanic for years and to day I cannot ingest!!!!  I insert the card, put in a folder name and click the blue ingest button and the folder appears in P/M but the images will not appear.
I have attached a screen shot of the ticked off items on the ingest panel that I have always used and hope that someone can see what has changed.

I injested images from another card using the same settings and it worked. I then tried the original card with Nikon Transfer and the images were downloaded without any problems. I erased all the images from the card, took some pictures and ingested the new images using the same settings as before and it worked fine???!!! 
Needless to say I am baffled as to what happened, maybe I need a new card.

Probably not the case, but you have incremental ingest turned on (so do I by the way), perhaps you had previously already ingested the images? (or PM thought toy had)
