Looks like you already have it doing just that, based on your screen grab.

The first three need to be under the hierarchial keywords but I am not sure if that is possible

I'm confused.  The first three items in your photo are hierarchical and are at the top of the list.  Are you talking about the "2 ocean lakes", "42 street", and "180 wea dental office equipment" items?  They don't look like they're hierarchical.  Since I'm just guessing at this point, if I'm wrong with either assumption, please list which items should be with the rest of the hierarchical items at the top of the list.

For similarly sized catalogs?

sorry for the confusion, the top three are actually within the WHO, WHAT and WHERE. Can I combine them into the larger ie structures folder that is already with WHAT

The keywords are just a representation of your actual keyword data.  So if you want the "people" to be under "WHO", then you need to change the keywords in your images to be WHO|people|... and then they'll show up under WHO.

It's not a virtual tree that allows rearranging.
