4.5 duplicating files during ingest

this is when ingesting from a Lexar firewire reader from a Lexar 133x speed 4 gb card, nef and jpg files from a nikon d2x, doesnt happen every time but when it does it duplicated the previous file and adds an A into the filename.

Are you renaming during Ingest?  Ingest automatically adds your choice of renaming collision styles whenever a file already exists at the destination.  Adding A, B, C, etc. is the default renaming collision style.

OK, then are you Ingesting into a folder that already has images in it?

That has been the case on some occasions and of course then it will use the collision renmaning system but its also occured on first download to an empty folder.

If you can tell me how to reproduce the problem then I would be happy to try and duplicate it and if I can, then we'll fix the problem.
