"Untitled" window

Why does a "untitled" window open continuously? Am I missing a preference option?

Yes.  You can control this in the Contact Sheet section of the Preferences dialog.

• "Create new Untitled window when resumed without any Contact Sheet window"

Uncheck the checkbox.

I have that unchecked yet it still creates an "untitled" window. Here is a screenshot attached...

Under what circumstances is the Untitled tab created?

Automatically, whenever I open Photo Mechanic, it is created. I can close it and then another one opens immediately in order like untitled1, untitled2, untitled3 etc........

Are you closing the tabs themselves or the top-level window?  If it is the tabs, there must always be at least one tab in each top-level window.  This is by design.

On the Mac version there must always be at least one tab open in any top-level contact sheet window.  When you close the last tab, a new one is created for you that you can use to browse a folder.

There is no preference that controls this behavior and it is by design.
