Is there a way for you to do a Search and Replace in Caption/Description field across multiple images.

Can I do a search in the database for say "John Smith" and then do a Search & Replace to make that now "John Smith IV"?

Yes.  First do a Search/Filter/Browse to produce the set of images that contain "John Smith".  Then use the Find and Replace command on the Edit menu to do the replacement.

The "searching the database" part is only in Plus, yes.  But the Find and Replace panel is available in both editions of the applications.
