Hi Kirk!
During the ingest from a SD-Card (Sandisk Extreme Pro, 8GB) using the built-in SD-Slot on my MacBook Pro, late 2011 I receive an errormessage:
Incremental Ingest: Unable to create or locate Ingest data file on /Volumes/EOS_DIGITAL. Is this a read-only volume? Incremental Ingest will not function for this volume.
Finished copying from volume "/Volumes/EOS_DIGITAL/".
Volume "/Volumes/EOS_DIGITAL/" has been unmounted and is safe to eject.
PM.log shows the following messages:
[Sat Dec-29-2012 15:41:40.918][b64a9000] CBFileIO.cpp:598 Could not open "/Volumes/EOS_DIGITAL/.pmingest.dat" for "wb" access. Sleeping 500ms and trying again.
[Sat Dec-29-2012 15:41:41.420][b64a9000] CBFileIO.cpp:598 Could not open "/Volumes/EOS_DIGITAL/.pmingest.dat" for "wb" access. Sleeping 500ms and trying again.

The SD-Card was formatted in the camera (Canon EOS 1D MK III) prior shooting. All images are ingested fine.
Ingesting the same card with the same images again using an external card reader works well without any errors. The same for the CF-Card used to save the same images as backup.
Using PM 5 Build 13507 (c69a93e) on OS X 10.8.2
Any ideas?
