Is there a way to have PM6 start and then ingest photos from a card reader when a card is inserted in the reader?

Without having PM6 running first?  It depends on whether your PM6 starts up when a card is inserted.  Does yours?  What OS do you run Photo Mechanic 6 on?  The instructions are different for macOS than Windows.

Without configuring PM6 to start up when a card is inserted, you can still auto-ingest by manually starting up PM6 and leave it running in the background.

Start PM6.  Open the Ingest dialog and get it set up with your desired options.  Set the "Auto Ingest" checkbox.  Click the "Close" button.

When you insert a camera card into your card reader, the Ingest will start a few moments later.

Have a look in the Extras folder in the disk image you used to install Photo Mechanic on your system.  In the Extras folder there are three other folders, one of which is the "PM Disk Launcher" folder.  Inside that folder is a ReadMe.txt file that you can read to learn how to add the PMDiskLauncherHelper to your system and get it to run in the background each time you start up your system.  Please let us know if you have any difficulties.
